Why do I need a medical history form?
It records your previous illnesses, medication and allergies. We need this information in order to treat you safely and to choose the right anaesthetic.
Can I fill in the form at home beforehand?
Yes, that is exactly why we provide it here as a PDF. Bringing it in already filled in saves time at reception.
Which form do I need for my child?
For dental treatment the children's dentistry form, for a brace treatment the orthodontics form for children. Adults use the corresponding forms for adults.
What do I do if I do not have a printer?
Then simply fill in the form on site – please come a few minutes earlier for that. Forms are available at reception.
